Monopolar Radiofrequency: Mechanism of Action

Thermage FLX delivers monopolar radiofrequency energy at 6.78 MHz through a capacitively coupled electrode. Unlike bipolar RF devices where current flows between two closely spaced electrodes (limiting depth to ~2mm), monopolar RF uses the patient’s body as a return path, allowing energy penetration to 4mm+ into the deep dermis and subcutaneous junction. This creates a larger, more uniform zone of volumetric heating at 65-75 degrees C. At this temperature, existing collagen fibrils undergo immediate denaturation and contraction (the instant tightening patients notice), while thermally damaged fibroblasts initiate a wound-healing cascade that produces new type I and type III collagen over the following months.

AccuREP Technology: 4th Generation Advancement

The FLX generation introduced AccuREP (Accuracy Repetition) — a proprietary algorithm that measures tissue impedance in real-time before each RF pulse and adjusts energy delivery parameters accordingly. Weiss et al. Clinical Efficacy and Patient Satisfaction

A systematic review by Beasley and Weiss (2014) analyzing 12 clinical studies with over 1,500 patients treated with Thermage found an average improvement score of 2.1 on a 5-point scale at 6-month follow-up, with patient satisfaction rates ranging from 72% to 91% depending on treatment area and baseline skin laxity.
