Forehead Anatomy: Frontalis, Corrugator, and Procerus Muscles

The forehead contains three muscles responsible for upper face wrinkle formation. The frontalis is a broad, thin muscle that spans the entire forehead and is the sole elevator of the eyebrows — it contracts when you raise your eyebrows in surprise, creating horizontal forehead lines. Without functional SNAP-25, acetylcholine-containing vesicles cannot fuse and release their contents into the synaptic cleft. The result is temporary chemodenervation: the muscle fiber receives no neural signal to contract and gradually relaxes. This effect lasts 3-6 months as the nerve terminal regenerates new SNAP-25 molecules and forms new functional neuromuscular junctions through a process called axonal sprouting. In the forehead, this selective muscle relaxation allows the overlying skin to smooth out, and existing wrinkle creases gradually soften as the collagen matrix remodels in the absence of repetitive folding.

Clinical Evidence for Forehead and Glabellar Botox

The landmark randomized controlled trial by Carruthers and Carruthers (2003) in the Journal of the American Academy of Dermatology demonstrated that botulinum toxin Type A achieved significant glabellar line improvement in 80% of treated patients at 30 days, compared to 3% in the placebo group. A subsequent multi-center study by Monheit et al. The concept of preventive or “baby” Botox has strong scientific rationale. Wrinkle formation progresses through three stages: (1) dynamic wrinkles visible only during muscle contraction, (2) static wrinkles visible at rest due to dermal collagen damage from repeated folding, and (3) deep creases with permanent structural changes to the dermal matrix. Once a wrinkle reaches stage 2-3, Botox alone cannot fully eliminate it because the collagen damage already exists. However, if Botox is initiated at stage 1 (dynamic wrinkles only), it prevents the mechanical folding that causes collagen breakdown, effectively halting the progression to permanent static lines. Blitzer et al. (2014) demonstrated in Aesthetic Surgery Journal that early Botox intervention required 30% lower doses and longer intervals between treatments compared to patients who started Botox after deep static wrinkles had formed.
